The Yellow Dress Girl is a mysterious NPC introduced in v0.18. She can be normally encountered in Deterioration, in a hidden area of the Apartments, or at the end of the Vomitgirl Area. The latter two require Sabitsuki to eat cake at the Sugar Hole a certain amount of times, each in a separate flow session, but without quitting the game, which will reset progress.
In Deterioration, the girl is found at the entrance, sitting and smoking. She will not react to any effects, though she can be killed with the Iron Pipe. One of the rooms down the hall also features her, sitting at a table as in the Apartments, with a Little Sabitsuki standing nearby.

To encounter her in the Vomitgirl Area, Sabitsuki must eat cake at the Sugar Hole five times, requiring 1,500¥. After doing so, interact with the maid with a hole in her face, found in the third room of the Sweet Sugar Hotel. This has a 1/5 chance of transporting Sabitsuki to the Vomitgirl Area, at the end of which the girl can be found overlooking a cliff. Because the path to her is blocked off by disembodied hands, it is impossible to interact with her.

To encounter her in the Apartments, Sabitsuki must eat cake at the Sugar Hole ten times, requiring 3,000¥. During the 11th visit, interacting with both Cleaners at the Sugar Hole before taking an elevator back to the Apartments will make a stairway previously blocked by a traffic cone available. In this new room, Sabitsuki and the girl in the dress can be found having tea before vanishing. To leave the area, Sabitsuki must wake up or use the Arm Effect. This event is often called "Sugar Float Days" by fans.

Theories
- Due to the great lengths that must be taken to encounter her in most areas, many believe that the girl is of importance to Sabitsuki in some way (such as being her mother, sister, a lover, or an unmasked Cleaner), and that there may be some trauma or tragedy involved.
- Because she is found at the edge of a cliff in the Vomitgirl Area, it is often believed that she committed suicide.
- Since the Sweet Sugar Hotel and Vomitgirl Area clearly suggest prostitution, it is possible that the girl was a prostitute herself.
